Occupational Therapist - Home Health

Join a home health team providing occupational therapy services to patients in their homes. In this role, you will evaluate patients, develop individualized treatment plans, and deliver therapeutic interventions that promote independence, improve functional abilities, and enhance quality of life. You will collaborate closely with physicians and interdisciplinary care teams to ensure optimal patient outcomes.

Key Responsibilities:

• Conduct in-home occupational therapy evaluations and ongoing patient assessments.
• Develop and implement individualized treatment plans based on physician orders and patient needs.
• Provide therapeutic activities, functional training, and environmental modification recommendations.
• Oversee the use of treatment modalities, prosthetic devices, and therapeutic equipment.
• Educate patients and caregivers to support safe and effective treatment outcomes.
• Collaborate with the interdisciplinary healthcare team to coordinate care.

Requirements:

• Graduate of an ACOTE-accredited Occupational Therapy program.
• Current Occupational Therapist license in the practicing state.
• Valid driver's license and automobile insurance.
• Current BLS certification.
• Mandatory Reporter certification for Child and Dependent Adult Abuse.
• Minimum of one year of experience with a similar patient population.
• NBCOT certification eligibility or certification highly preferred.

Schedule: Monday - Friday, 8:00 AM - 4:30 PM
