Wound Care & Ostomy RN Specialist — Lead & Educate

Parkland Community Health Plan (PCHP) seeks an experienced Registered Nurse (RN) with 2+ years in wound ostomy care to coordinate specialty patient care using a dedicated practice model. The RN will assess, plan, implement, and evaluate care with a focus on patient transitions and preventing delays or complications.
