RN - Case Manager

Case Manager RN
Responsibilities include providing care coordination and management of patient care. Assess patient needs and implement appropriate interventions. Collaborate with multidisciplinary teams to ensure optimal patient outcomes. Utilize critical thinking skills to develop and monitor care plans. Maintain accurate documentation in compliance with policies and regulations. Participate in quality improvement and educational initiatives. Expected to work every other weekend and holidays in rotation with facility employees. Flexibility to adapt to changing patient care needs is essential.
Minimum 1 Year Experience License: Current RN License Certifications: BLS Certification
