Job Title: RN - Care Manager - ED
Location: Bronx, NY
Type: Contract | 14 Weeks
Shift & Schedule: Monday–Friday | 9:00 AM – 5:00 PM | 35 Hours Per Week

Position Overview:

Navitas Healthcare, LLC is seeking a RN - Care Manager - Emergency Department (ED) for an exciting Travel or Local job in Bronx, NY.

Responsibilities

• Coordinate and manage patient care plans throughout the Emergency Department stay.

• Conduct comprehensive patient assessments to identify medical, psychosocial, and discharge needs.

• Collaborate with physicians, nurses, social workers, and interdisciplinary healthcare teams to ensure optimal patient outcomes.

• Perform utilization review activities to ensure appropriate level of care and resource utilization.

• Develop and implement discharge plans and coordinate post-acute care services.

• Facilitate communication between patients, families, healthcare providers, and external agencies.

• Monitor patient progress and adjust care plans as needed to support quality care and efficient transitions.

• Ensure compliance with hospital policies, regulatory requirements, and case management standards.

• Maintain accurate and timely documentation within electronic medical records and care management systems.

• Participate in quality improvement initiatives and patient-centered care programs.

• Educate patients and families regarding treatment plans, discharge instructions, and available support services.

• Support departmental goals related to patient flow, length of stay reduction, and readmission prevention.

Required Qualifications

• Active Registered Nurse (RN) License in the State of New York.

• Minimum five (5) years of clinical nursing experience.

• Experience with care coordination, utilization review, and discharge planning.

• Associate Degree in Nursing (ASN or ADN) from an accredited nursing program.

• Current Basic Life Support (BLS) Certification.

• Excellent communication, organizational, and critical thinking skills.

• Experience working in an Emergency Department or acute care environment preferred.

• Case Management Certification preferred.
