Description:
We are looking for a Physical Therapist to support the treatment of illness and/or injury through the use of physical exercises and therapies.
This role assists in the rehabilitation and restoration of body functions to promote client wellness and independence.
You will provide therapy programs for patients with developmental deficits, physical injury, or illness.
Other responsibilities and duties may be assigned based on facility needs.


Responsibilities include:
- Coordinating therapy services and providing direct therapeutic care to assigned clients.
- Teaching rehabilitation skills to clients and caregivers to promote maximum independent functionality and safety.
- Providing preventive and rehabilitative procedures individualized to clients according to age-specific needs.
- Performing duties within the scope of physical therapy practice guidelines.
- Developing and revising plans of care based on initial assessments and continual reassessments of client needs, histories, diagnoses, and goals.
- Monitoring the application of treatments involving physical agents such as heat, water, and electricity.
- Following utilization guidelines and protocols.
- Communicating significant clinical findings and changes to the relevant personnel.
- Documenting all skilled and medically necessary care thoroughly, timely, and accurately.
- Ensuring compliance with Medicare, State, Federal regulations, and local policy.
- Maintaining client and organizational confidentiality.
- Participating in quality management programs and completing required in-service training.
- Supervising Physical Therapy Assistants and Home Health Aides as applicable.
- Participating in staff meetings and case conferences as required.
- Delivering a high level of customer service to internal and external customers.
- Demonstrating leadership competencies through communication, relationship building, and teamwork.
