VillageMD is seeking a RN Care Manager to collaborate with providers and engage high-risk patients in developing patient-centered care plans. You will work with care coordinators, nurse navigators and social workers to connect patient care across the continuum. You will operate 2-3 days per week in physician offices, coordinating education, referrals and action steps to improve wellness and reduce unnecessary utilization, while maintaining a patient-first approach. #J-18808-Ljbffr shm
