Role Description This is a part-time, on-site Physical Therapist role based in Crystal Lake, IL. The Physical Therapist will evaluate patients’ physical function, mobility, and pain levels, and develop individualized treatment plans that support rehabilitation goals. Day-to-day responsibilities include delivering evidence-based therapeutic interventions, supervising therapeutic exercises, and providing education on home exercise programs, injury prevention, and self-management strategies. The role involves close collaboration with other healthcare professionals, regular documentation of progress notes and treatment outcomes, and timely communication with patients, caregivers, and referring providers. The Physical Therapist will contribute to a supportive clinic environment that promotes safety, respect, and high-quality patient care. Qualifications Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and adjust individualized treatment plans. Demonstrated skills in Patient Care and Home Care, including patient and caregiver education and safe progression of exercises. Working knowledge of Medicine principles relevant to musculoskeletal, neurological, and functional conditions treated in outpatient settings. Current state licensure as a Physical Therapist (or eligibility to obtain licensure in Illinois). Doctor of Physical Therapy (DPT) or equivalent accredited degree. Experience in outpatient or home health rehabilitation is preferred. Ability to perform safe patient handling, including transfers and mobility assistance as needed. Excellent communication, documentation, and time-management skills, with a patient-centered, culturally sensitive approach. #J-18808-Ljbffr
