Licensed Practical Nurse assists physicians with patient care and follow-up providing direct and indirect patient care in a clinical setting. Duties to include, but not limited to, interviewing patients, obtaining vital signs, preparing patients and assisting physicians with patient exams and/or treatment. Continuing responsibilities including charting and clerical work as necessary to oversee phone prescriptions and refills to pharmacies. Additionally, the LPN will perform triage with patient phone calls to identify emergent and non-emergent situations. LPNs will have the opportunity to perform injections under the direction of the physician and supervising Registered Nurse.

Requirements:

• Licensed Practical Nurse.
• Three years ambulatory care experience preferred.
• Current active Idaho state licensure.
• Current BLS certification or the ability to get certified within one month of hire.

Knowledge, Skills, and Abilities Required:

1. Medical Office experience required.
2. Knowledge of FQHC's preferred

Duties and Responsibilities:

1. Participates in preparing patients for clinic appointments
2. Patients are brought to exam room upon their arrival to clinic
3. Administer direct hands on patient care under the supervision/direction of supervising Provider/registered nurse.
4. Obtains a complete set of vital signs including a pain score at each visit.
5. Ensure patient flow is timely and accurate throughout the clinic
6. Reviews and documents patient medications
7. Checks equipment daily to ensure accurate functioning
8. Assist providers/medical assistants with emergency care of patients in the clinic
9. Handles acute treatment or crisis intervention
10. Heritage Health staff have an active role in our Patient Centered Medical Home model of care. This role is designated as part of the Heritage Health PCMH Care Team.
11. Regular and predictable attendance is an essential function of this position.
12. Performs all other duties as requested.
13. Ensure all work is OSHA and HIPAA compliant.

Physical/Mental Requirements:

Office staff is required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands or fingers to handle or feel over two-thirds of the time; while reaching with hands and arms occurs one-third of the day. Climbing or balancing, stooping, kneeling or crouching occur less than one-third of the time. Communicating by talking/hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 – 40 pounds. Rarely is there a need to lift more than 41 pounds. Must be able to manage efficient keyboard use, hear, see, speak and read.

Working Conditions:

Work is normally performed in a typical interior/office work environment in a clinic. Work may be demanding and chaotic at times. Exposed to patient population that will present a variety of contagious diseases, physical injuries, and emotional states of mind.
