Requirements
- Current state license as a Registered Nurse
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- Homecare Homebase Software experience
- OASIS experience
- CHPN certification (preferred)
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance

Roles & Responsibilities
- Assess home health and hospice patients to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families regarding care, medications, and safety, promoting self-care practices.
- Supervise and provide clinical direction to assistive personnel to ensure quality service delivery.
- Perform on-call responsibilities and provide effective communication to patients, families, and healthcare teams.
- Attend interdisciplinary team meetings and other necessary meetings as required.

Compliance / Notes
- Must comply with OSHA infection control measures.
- Maintains current licensure and continuing education requirements.
- Ability to work a flexible schedule and travel locally.
- Confidentiality must be maintained.
