*Job Summary: *We are seeking a friendly and compassionate Pulmonary Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive pulmonary care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. *Responsibilities:*

* Conduct pulmonary assessments for patients in post-acute care settings
* Evaluate, diagnose, and manage pulmonary conditions including COPD, Asthma, and more.
* Evaluate, treat, and manage patients experiencing acute or worsening medical conditions requiring timely intervention
* Evaluate, diagnose, and manage pulmonary conditions including COPD, asthma, pneumonia, and respiratory failure
* Develop and implement individualized treatment plans
* Collaborate with interdisciplinary teams
* Educate patients, families, and facility staff on respiratory care and treatment plans
* Monitor patient progress and adjust care plans as needed
* Maintain accurate and timely EMR documentation
* Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events
* Experience with ventilator management is preferred.

*Requirements*

* Preferred a minimum of 6 months of experience in post-acute care setting
* Preferred experience in Pulmonary: training provided
* Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required
* Masters Degree in Nursing from an accredited Nurse Practitioner program or Masters Degree from an accredited Physician Assistant program required.
* CPR BCLS/BLS certified
* Strong communication, organizational, and teamwork skills
* Ability to work independently in the clinical environment
