Social Worker

We're seeking a compassionate and motivated Social Worker to join our dedicated team. In this role, you’ll play a key part in supporting the emotional and psychosocial well‑being of our residents, working closely with families, interdisciplinary team members, and community partners.

Social Worker Responsibilities:

• Assist residents and families in adjusting to short-term rehab or long-term care placement
• Conduct psychosocial assessments and care planning
• Coordinate discharge planning and referrals
• Advocate for residents’ rights and help resolve concerns
• Participate in care plan meetings and interdisciplinary team discussions
• Maintain accurate documentation in compliance with state and federal regulations

Social Worker Requirements:

• Bachelor’s or Master’s degree in Social Work (BSW or MSW)
• Prior experience in long-term care or healthcare setting preferred
• Strong interpersonal, organizational, and problem-solving skills
• Knowledge of federal and state regulations related to nursing facilities
• Empathetic, professional, and resident-centered approach
