RN Care Manager – Hybrid (Hammond, LA) Job ID: 2360437 As part of the Clinically Integrated Network, the RN Care Manager engages in a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet a patient’s health needs through communication and available resources, promoting quality, cost-effective outcomes. The RN Care Manager collaborates telephonically with clinical teams across various health care settings – acute care, skilled nursing, long‑term acute care, rehabilitation, custodial care, and ambulatory care – to facilitate co‑ordination of care, resulting in more efficient utilization of health care resources, reduced hospital readmissions, and improved health outcomes. THIS IS A HYBRID ROLE. YOU WILL WORK THREE DAYS IN THE CLINIC AND TWO DAYS IN A VIRTUAL CAPACITY. Primary Responsibilities Discharge Planning: Works with facility case manager or social worker to coordinate discharge process and transition needs based on patient’s diagnoses and status. Care Coordination and Patient Support: Collaborate with the entire clinic team to proactively manage high‑risk patients by ensuring appropriate: Telephonic post‑hospital discharge calls Medication management, including medication reconciliation post‑discharge and medication adherence Chronic disease education Provide self‑management support to patients to improve health outcomes. Clinic Team Communication: Monthly in‑person or virtual meetings where patients’ goals, outcomes, interventions, and recommendations for improvement are discussed with members of the patient’s care team. Travel: 30% of the time within assigned territory. At least 3 years of experience in a clinical setting. Experience with Microsoft Office (Word, Excel, Outlook). Thorough knowledge of HIPAA compliance. Ability to travel: 30% of the time within assigned territory. Driver’s license and access to reliable transportation. Preferred Qualifications Current CPR certification. Experience with electronic medical records (EMR). Experience in disease management, chronic care improvement, and/or quality improvement in a managed care setting. Physical Demands/Working Conditions Ability to travel by personal vehicle to providers’ offices on a routine basis. Availability to work extended hours, overtime, and non‑traditional hours as needed. Pay and Benefits Pay is based on several factors including local labor markets, education, experience, and certifications. Salary for this role will range from $60,200 to $107,400 annually for full‑time employment.
