Physical Therapist - Travel Contract

Seeking a travel Physical Therapist to provide home health services in Oak Brook, IL and the Central Metro E coverage area. The clinician will deliver adult and geriatric general med/surg care, perform skilled PT visits in patient homes, support ADL and adaptive equipment training, and collaborate with interdisciplinary team members virtually. Orientation will include approximately two days onsite in Oak Brook for EMR and skills check-off, after which work is conducted remotely with travel to patient homes within the designated coverage area. Candidate must provide reliable transportation and maintain required licensure and certifications.

Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois PT license required prior to assignment start.
• Minimum 2 years recent Home Health physical therapy experience with adult/geriatric patients. (Client will consider 1 year if the candidate demonstrates recent, relevant home health experience.)
• Current BLS certification from AHA or ARC.
• Valid driver’s license, active auto insurance, and reliable personal transportation to travel to patient visits.
• Experience with EPIC EMR and OASIS documentation.
• Ability to assist with ADLs and adaptive equipment and to collaborate effectively with RN, OT, SLP, and other team members.

Preferred Qualifications

• Home health travel experience.
• Master’s degree in Physical Therapy.

Assignment Details

• Orientation: Approximately 2 days onsite in Oak Brook, IL for EMR and skills check-off.
• Coverage Area: Central Metro E including ZIPs 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas.
• Schedule: Monday–Friday, 8:00 AM – 5:00 PM schedule cycle.
• Patient Load: Expectation is approximately 6 patient visits per day within the coverage area.
• Work Location: Remote/field-based home health. No on-site office; meetings are held virtually.
• Transportation: Traveler must provide own transportation to/from all patient visits.
• Attire: Solid color scrubs or business casual. No jeans or open-toe shoes.

Impacting Patient Care

The travel PT will support a home health census serving adult and geriatric medical-surgical patients. The role requires timely, quality documentation in EPIC, interdisciplinary collaboration, and delivery of safe, effective functional interventions to enable patient independence in the home environment. Orientation and supervisory resources are provided by nursing leadership and team leads.
