Restorative Care Lead - Rehab Collaboration & QA

Alden seeks a Restorative Nursing Coordinator to lead the restorative nursing program across the facility. You will act as liaison between formalized therapy and nursing, promoting a restorative philosophy to maximize residents’ function.

Responsibilities include directing restorative services, maintaining documentation, coordinating with Nursing, Dietary and Therapy, and performing functional assessments. Illinois RN/LPN license, restorative certification, and CPR required.
