Nurse Practitioner

Your Role & Responsibilities Provide comprehensive care to seniors following clinical standards. Collaborate daily with your care team to ensure coordinated, high-quality care. Manage referrals, hospital/SNF coordination, DME, and home health services. Participate in daily huddles. Document care with support from quality-based coders. Participate in on-call rotation. Support growth initiatives at your center. Ensure compliance with all licensing and accreditation requirements. Qualifications Required: Master’s Degree as a nurse practitioner or completion of a physician assistant program with board certification. Active, unrestricted NP or PA license in the state of practice. Two years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions. Computer proficiency (EMR, email, etc.). Patient‑facing role part of Humana/Senior Bridge’s Tuberculosis (TB) screening program; required to be screened for TB. Preferred: Experience in Medicare Advantage settings. Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them. Familiarity with HEDIS quality indicators and Medicare guidelines. Compensation Pay Range: $123,800 - $170,300 per year. Job is eligible for a bonus incentive plan based upon company and/or individual performance.
