Registered Nurse Hospice Hospital LiaisonLocation: Monmouth CountySchedule: Full TimePosition Summary: The VNA Health Group is seeking a compassionate and experienced Registered Nurse Hospice Liaison to join our dedicated Hospice Team. This vital role focuses on assessing patients for hospice eligibility, coordinating their transition to hospice care, and conducting patient admissions. The Registered Nurse Hospice Liaison will be responsible for covering the following hospitals: Monmouth Medical Center and CentraState Medical Center.
