Full Time Professional Essex Junction, VT, US 4 days ago Requisition ID: 1585 Salary Range: $117,268.00 To $143,328.00 Annually CHC is looking to add either a Nurse Practitioner or Physician Assistant to our team. Nurse Practitioners and Physician Assistants (NP/PA) are clinical leaders of the care team that are responsible for providing direct outpatient medical services to CHC patients in support of the organizational mission. This role renders services that reflect current standards of medical care appropriate to the local medical community. They are responsible for coordination and documentation of continuity of care and for risk management for patients. The NP/PA role participates in quality assurance and quality improvement efforts, participates in lifelong learning, and supports the financial stability of the organization to continue the provision of high quality and accessible care at CHC.

BASIC QUALIFICATIONS

Graduation from program of study accredited by the appropriate professional board. Ability to work independently and solve problems creatively with initiative. Affinity for working with a culturally and economically diverse patient population. Good organizational and interpersonal skills. Willingness to work flexible hours. Licensure for practice in the State of Vermont. Board certified or eligible for board certification. Active DEA license. CPR certification. Completion of requirements for successful credentialing and privileging by CHC and applicable insurance companies.

ESSENTIAL DUTIES

A. DIRECT PATIENT CARE & SERVICES

Provide direct outpatient medical services across a range of practice areas commensurate with board certification. Ensure timely medical, laboratory, and diagnostic follow‑up and documentation in the EHR. Meet productivity expectations, see patients in available schedule time, and increase panel size by providing access to new patients. Communicate effectively and timely with patients, empower them to take an active role in health maintenance, and promote wellness behaviors. Complete all clinical documentation promptly and in accordance with CHC policies. Refer patients to appropriate specialty care and ensure proper communication and follow‑up. Participate in CHC’s 24‑hour on‑call coverage system. Follow CHC clinical policies and procedures.

B. CLINICAL SUPERVISION

Seek clinical supervision when needed, including case consultation and internal supervision. Provide clinical supervision for nursing staff as assigned. Teach trainees such as medical, nursing, NP, PA students and resident physicians as assigned.

C. QUALITY ASSURANCE / QUALITY IMPROVEMENT

Participate in quality assurance activities such as development of flow sheets, data collection instruments, chart audits, and case conferences. Participate in interdepartmental quality improvement teams. Participate in efforts to improve quality outcomes defined by non‑CHC entities. Contribute to CHC’s quality improvement efforts, including cost‑effective care for patients and improved staff quality of work‑life.

D. CONTINUING EDUCATION

Participate in continuing education required for licensure and board certification; submit documentation to supervisor. Develop and present continuing education sessions for CHC staff. Attend staff and team meetings as assigned.

E. COMMUNITY PRESENCE

Communicate a positive image of CHC to the community.

Work in an environmentally controlled office/health clinic setting. Ability to take overnight call in addition to daily clinical responsibilities. Ability to lift/push/pull up to 35 lbs.
