• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.
• Facilitate clients in achieving personal independence and assist in developing daily living skills.
• Perform other duties as assigned.
• Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates. Supervision Given: This position does not supervise any staff. Supervision Received: The SUD Case Manager reports to their assigned Substance Operations Managers. Qualifications:
• Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.
• Knowledge of behavioral health practices and community resources.
• Ability to work independently and as part of a team. Education:
• High school diploma or equivalent is required;
• a college degree in Human Services is preferred. Certification:
• CPR certification is preferred.
• NPI number/Ohio Medicaid ID is required (instructions on how to apply will be provided). Experience:
• Minimum of 3 years in behavioral health or a related field is preferred, but not mandatory. Physical Effort:
• Requires sitting for extended periods and occasional lifting of up to 20 pounds. Background:
• Must pass all Federal and State background checks according to company policy. Schedule:
• Flexible scheduling options allow for accommodating personal availability, enabling as little or as much work as desired. Benefits:
