Vista Life Innovations in Madison, CT, seeks a Case Coordinator (Case Manager) to deliver direct support and case coordination to Engage members. You will manage a caseload and serve as the primary contact for members, families, guardians, and partners, with ~30 hours of direct service weekly and remaining time on admin, documentation, meetings and family updates. Requires a BA plus one year of related experience, a valid driver’s license, CPR/First Aid and PMT certifications (or ability to #J-18808-Ljbffr Greater Old Saybrook Chamber
