Advanced Practice Provider - Nurse Practitioner (TRAVEL) Position Summary: The Advanced Practice Provider (APP) plays a pivotal role in coordinating the continuum of care and managing the overall patient experience within the USC Brain Health Network (UBHN), ensuring good communication, efficient and patient-centered operations, and effective service delivery for every patient. They are responsible for coordinating and navigating the care for all patients referred to the UBHN for services between the primary care provider and referrals to ancillary services and specialties. To demonstrate commitment to conducting business lawfully and ethically by consistently adhering to compliance policies, procedures, and regulations. This position will travel across the state of South Carolina. Qualifications Requires at least a bachelor’s degree in a related field and 3 or more years of related experience. Substitutions may be made for equivalent combinations of certification, training, education, and/or experience. Current licensure as a Nurse Practitioner with SC LLR, in good standing. Preferred: At least one year of experience working in a physician practice. Preferred: Experience coordinating care of patients across different areas/services of a health system. Knowledge of medical terminology, use of medical equipment, and ability to follow clinical orders. Ability to communicate effectively, assess, identify, and address individual needs. Responsibilities Serve as the primary point of contact for all primary care patient referrals within the UBHN, taking calls from providers/offices and connecting with patients. Work with referred patients to ensure that all clinical services are received timely and appropriately, and results are communicated within the UBHN following established protocols. Follow up with patients to provide appointment reminders and to ensure patient comprehension and compliance. Work collaboratively with assigned physician(s) to regularly discuss care and management of assigned patients. Accept and perform other related duties as assigned.
