Hospice Registered Nurse (RN) - Days

Location: Moab, Utah

Assignment Duration: This is a 13-week contract position.

Schedule: Days with an on-call rotation of 1-3 shifts per week, including 3 weekends during the contract.

Job Description: We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team, providing skilled nursing care and support to terminally ill patients and their families.

Responsibilities

Perform comprehensive assessments of patients' needs and coordinate individualized care plans.
Provide pain and symptom management consistent with the hospice plan of care.
Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy.
Collaborate with the interdisciplinary team including physicians, social workers, chaplains, and volunteers.
Maintain accurate and timely documentation in compliance with state, federal, and organizational policies.

Required Experience / Certifications / Licensure

Current Registered Nurse (RN) license in the state of Utah or compact state RN license.
Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
Current BLS Certification.
Valid driver's license and reliable transportation required for home visits.
