MDS Coordinator - LVN - 7255US:CA:Santa Rosa | Nursing & Clinical | Full Time, Hourly, 36+ hours a week55.25 - 60.00 USD per hourPosted todayDescriptionSpring Lake VillagePay Range: $55.25 - $60.00Shift: Tuesday - Saturday, 8:30 AM - 5:00 PMFull Time with Benefits.Summary: Under the direction of the DON, the MDS/RAC is responsible for conducting and coordinating the completion of the resident comprehensive assessment (RAC) which includes the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and the RAP summary sheet. The coordination of periodical comprehensive, interdisciplinary assessments upon admissions, annually and whenever there is significant change of condition.Detailed responsibilities:MDS Coordinator - Provides quality Customer Service efficiently to residents, families, co-workers and vendors in a manner to ensure satisfactionEnsures interdisciplinary, comprehensive assessments and care plans are completed using common assessment periods within the times frames established by OBRA regulationSchedules interdisciplinary conference for review and updating of resident assessment and care planMonitors plans of care to ensure resident assessments are utilized in the development of an interdisciplinary plan of care reflective of the resident's current physical, mental and psychosocial needsProvides educational training and clinical supervision in resident assessment for appropriate health care workersAssists Resident Assessment Coordinators (MDS) with computerized medical data and transmissionEnsures documentation relative to resident assessment process (RAI) is maintained according to State and Federal regulations and company policy and procedureCommunicates problem identification with resident assessment process promptly to the DON and Executive DirectorConducts and participates in PDPM meetingsReports data collection, trends, QSAP reports and attends QA meetingsPerforms all duties in a safe and efficient manner Wears safety equipment as required for the job Reports any safety hazards and/or accidents to supervisor - Attends all mandatory in-service meetings Complies with all department and facilities policies and procedures - Front Porch is committed to the prevention of accidents by providing equipment with safeguards, personal safety equipment on jobs which require it, and adequate safety instructions to all employees - All employees are required to follow safe work habits in order to prevent injuries to themselves, residents, visitors or other employeesCompetencies and skills: Essential: Must have good communication and customer service skills; ability to work well with others and take direction; ability to work under pressure to ensure timely review, correction and reporting of the payroll.
