Job Detail: Job Title: Physical Therapist - Home Health Location: Easton, Maryland [Coverage Areas: Caroline, Queen Anne, Talbot] Schedule: Monday – Friday, 8:00 AM – 4:30 PM Duration: 13 Weeks Local Pay: $55.00 - $60.00/hour (W-2) Plus Mileage reimbursement *Negotiable & Pay offered is based on experience, expertise, credentialing, and education. Weekly Hour Guarantee: 40 hours On-Call/Weekend Requirement: None Mileage Reimbursement: Yes (from first patient to last patient) Key Responsibilities: Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team Participate in case conferences, care coordination activities, and team collaboration Ensure compliance with CMS regulations, infection control standards, and agency policies Manage a mobile caseload efficiently while meeting productivity and documentation expectations Adapt quickly to new environments, workflows, and patient populations as a travel clinician Required Qualification/Certification Graduate of an accredited Physical Therapy program Active Physical Therapist license in the state of assignment At least 1 year of home health experience and experience with HCHB Willingness to travel within the assigned coverage area Current BLS certification Valid driver’s license.
