Nurse Practitioner or Physician Assistant

Community Care Physicians, P.C., a leading multi-specialty medical group with over 1800 employees, is seeking a compassionate and highly skilled Nurse Practitioner or Physician Assistant to join our Healthcare & Medical Services department. In this role, you will provide comprehensive, patient-centered care in a collaborative, team-based environment that emphasizes quality, innovation, and positive patient outcomes. Working within our diverse, multi-specialty network, you will perform detailed patient histories and physical examinations, formulate differential diagnoses, order and interpret diagnostic tests, and develop evidence-based treatment and management plans for patients across the lifespan or within a defined specialty. You will be responsible for prescribing medications and therapies in accordance with state regulations and clinical guidelines, managing both acute and chronic conditions, and coordinating care with physicians, specialists, nurses, and other members of the care team. You will play a key role in educating patients and families, promoting preventive care and wellness, and supporting shared decision-making to improve overall health and satisfaction. Using our Electronic Health Record, you will document all encounters accurately and efficiently, ensuring compliance with regulatory standards and contributing to the practice’s quality and population health goals.
