Physical Therapist – Rehabilitation

Schedule: Day Shift

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 36 hours per week

Requirements:

• Current CA Physical Therapist License
• Basic Life Support (BLS) certification
• Driver's License
• Auto insurance
• Auto registration
• Tuberculosis (annual) test
• MMR vaccination
• Varicella vaccination
• Hepatitis B vaccination
• Tdap vaccination (valid for 10 years)
• Annual flu vaccination
• Drug screen clearance
• Physical

Roles & Responsibilities:

• Evaluate and treat patients with physical impairments or disabilities.
• Develop individualized therapy plans based on patient needs and goals.
• Implement therapeutic exercises and treatments to enhance physical function.
• Monitor patient progress and adjust treatment plans accordingly.
• Educate patients and families on rehabilitation strategies and home exercises.
• Document patient interactions and progress in accordance with facility policies.

• All requested time off must be disclosed at the time of submission.
• Time off requested after acceptance will not be approved.
