Responsibilities As a Primary Care Physical Therapist, you will perform visits in homes, assisted living facilities, inpatient rehabilitation facilities, clinics, and via Telehealth. Responsibilities include obtaining complete health and psychosocial history, performing physical assessment, providing health education and counseling, arranging referrals, utilizing appropriate reporting mechanisms, respecting patient privacy, obtaining authorization and consents, reviewing medical diagnosis and treatment plans, and providing community education.
