Licensed Practical Nurse (LPN) Job Summary

Rose View Rehab and Care Center is seeking a highly skilled, compassionate Licensed Practical Nurse (LPN) to join our dedicated healthcare team for Full-Time, Part-Time, or PRN shifts. Under the supervision of Registered Nurses (RNs) and medical staff, you will provide exceptional direct patient care to ill, injured, or convalescing residents in our long-term care and rehabilitation facility. Key duties include monitoring vital signs, administering prescribed medications, dressing wounds, charting patient status, and ensuring resident safety and comfort. Licensed Practical Nurse (LPN) Duties and Responsibilities

• Assess, record, and maintain accurate patient medical histories, clinical charting, and vital signs (blood pressure, pulse, temperature, oxygen levels) in Electronic Health Record (EHR) systems.
• Administer prescribed oral, topical, and injectable medications; track dosage schedules, monitor patient responses, and log all treatments accurately.
• Perform routine wound care, change dressings, manage bandages, maintain catheters, and manage feeding tubes as ordered by physicians.
• Setup intravenous (IV) therapy lines and administer IV medications if state-certified and within regulatory scope.
• Assist patients with activities of daily living (ADLs), including personal hygiene, bathing, dressing, mobility, and nutritional intake.
• Collect biological specimens (blood, urine, swabs) and deliver them to designated laboratories for diagnostic testing.
• Monitor residents for notable behavioral or physical changes, report drastic symptom shifts to supervising RNs or physicians, and assist in medical emergencies.
• Sterilize medical equipment, sanitize treatment operatories, and maintain compliance with infection control, HIPAA, and safety regulations.
• Provide emotional support, patient education, and compassionate communication to residents and their families.

Licensed Practical Nurse (LPN) Requirements and Qualifications

• Completion of an accredited, state-approved Licensed Practical Nursing (LPN) or Licensed Vocational Nursing (LVN) diploma/program.
• Active, unencumbered Licensed Practical Nurse (LPN) state license in good standing.
• Current Basic Life Support (BLS) and CPR certifications required; IV therapy certification strongly preferred.
• 1+ years of clinical nursing experience in a skilled nursing facility (SNF), long-term care center, rehabilitation hospital, or acute care setting preferred.
• Solid understanding of state healthcare standards, patient care procedures, pharmacology, and medical terminology.
• Proficiency with Electronic Health Record (EHR/EMR) software and medical data entry.
• Strong clinical observation skills, high attention to detail, physical stamina for long shifts, and excellent interpersonal communication.
