Rural Health Access Navigator for Community Care
Scotland County Hospital
Scotland County Hospital is seeking a Full-Time RHTP Navigator to serve as a frontline, community-based member of the local hub. You will perform standardized screening, intake, and warm handoffs across Hub Partners to connect residents with essential services. You will manage an assigned caseload, document activities in EHR or CIE systems, and report outreach outcomes. Travel within the multi-county area, strong communication, and a high school diploma with CHW training are required; bilingual #J-18808-Ljbffr Scotland County Hospital
