• Resident Advocacy: Serve as a trusted advocate, ensuring residents' rights and dignity are upheld.

• Individualized Care Planning: Collaborate with interdisciplinary teams to develop and implement comprehensive care plans tailored to each resident's unique needs.

• Emotional Support: Provide counseling and support to residents and families during times of transition or crisis.

• Resource Coordination: Connect residents and families with community resources, benefits, and support services.

• Compliance Excellence: Maintain compliance with all federal, state, and local regulations, including participation in surveys and audits.

• Team Leadership: Supervise and inspire the social services team, fostering a culture of compassion and professionalism.

• Family Engagement: Build strong relationships with families, offering guidance and open communication to ensure their loved ones’ needs are met.

• Bachelor’s or Master’s degree in Social Work or a related field (LCSW preferred).

• Minimum 2 of experience in social work, preferably in a Skilled Nursing Facility or healthcare setting.

• Strong knowledge of state and federal regulations (e.g., MDS, PASRR, etc.).

• Exceptional communication, organizational, and problem-solving skills.
