Home Health Clinician (PT) - Indianapolis, IN Surrounding Area Schedule: Monday-Friday, 8:00 AM-4:30 PM Weekly Hour Guarantee: 36 Hours Productivity Expectation: 25-30 Points Per Week Coverage Area: Indianapolis and Surrounding Areas On-Call/Weekend Requirement: None Mileage Reimbursement: Yes, mileage is reimbursed between patient visits (from first patient to last patient). Position Overview We are seeking an experienced Home Health Clinician (RN, PT, or OT) to provide high-quality patient care throughout the Indianapolis surrounding area. The ideal candidate will have prior home health experience and a strong commitment to delivering compassionate, patient-centered care in the home setting. Key Responsibilities Complete Start of Care (SOC) assessments and admissions. Perform patient recertifications in accordance with regulatory and organizational requirements. Conduct patient discharge visits and complete all related documentation. Assess patient needs and develop appropriate care plans. Coordinate care with physicians, interdisciplinary team members, patients, and families. Maintain accurate and timely clinical documentation. Provide patient and caregiver education to support positive health outcomes. Ensure compliance with all clinical, regulatory, and agency standards. Required Qualifications Active licensure as a Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT). Previous Home Health Experience Is Required. Strong clinical assessment and critical thinking skills. Reliable transportation and ability to travel throughout the assigned coverage area. Preferred Qualifications Strong organizational and time management abilities. Ability to work independently while collaborating effectively with the care team.
