Job Summary and QualificationsRole Overview:
Under the guidance of the Operations Director, you will plan, organize, and implement in-home services for PACE participants and their families. Your responsibilities will include, but are not limited to:
- Coordinating daily operations of In-Home Services for your team of participants.
- Acting as the liaison between the contracted home health agency and the PACE program, managing homecare nurses, home health aides, non-skilled aides, and homecare schedulers.
- Utilizing nursing skills to assess participants in their home settings, coordinating care plans with appropriate resources, and providing treatments and health education as needed.
- Participating in an interdisciplinary team to ensure compliance with competency requirements, maintaining medical records, and thorough documentation.
- Planning and utilizing telehealth, assistive technology, and community resources to implement health and wellness initiatives in homes.
Skills, Knowledge, and Abilities:
Required Education:
A Bachelor of Science in Nursing is required. Nurses with an Associate Degree or Diploma must agree to obtain a BSN within 6 years of starting in this position. Nurses nearing the end of their careers may be exempt from this requirement with approval from the CNO System Council.
Required License:
- Must hold and maintain current licensure as a registered nurse with the North Carolina Board of Nursing. A compact license may apply; licensees should verify with NCBON.
- Must have a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” showing $100k/$300k Bodily Injury/$50K Property Damage coverage), and have reliable transportation.
Required Experience:
- At least two (2) years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting.
- A minimum of one year working with a frail or elderly population.
- A willingness to work in patients' home environments, respecting their values and standards.
Preferred Experience:
- Proficiency with EMR systems and familiarity with telehealth and video consultations.
- Three years of nursing experience in a community home-based setting is preferred.
- Two years of experience as an ACTT RN or in Mental Health.
