Job Title: Clinical Pharmacist
Job Description
This onsite Clinical Pharmacist role focuses on providing high‑touch, patient‑centered care by performing comprehensive pharmacotherapy reviews, counseling patients by phone for most of the day, and ensuring safe, compliant dispensing practices. The pharmacist supports a specialized patient care team, oversees pharmacy staff, and coordinates closely with clinicians and caregivers to simplify access to medications and help patients achieve optimal outcomes. The position requires living within one hour driving distance of a Kentucky facility and working primarily Monday through Friday, 9:00 a.m. to 6:00 p.m. Eastern Time during training, with ongoing responsibilities that may include on‑call coverage.
Responsibilities
- Conduct comprehensive clinical reviews of pharmacotherapy for each patient, including drug utilization, adherence counseling, compliance assessment, drug interaction screening, side effect profiling, and drug administration support to ensure treatment plans align with evidence‑based recommendations.
- Document all pharmacotherapy reviews and clinical interventions clearly and accurately in the patient chart so that nursing and other care team members have the information needed for follow‑up assessments.
- Provide detailed counseling to patients on new and ongoing drug therapies in accordance with clinical management models, ensuring patients understand their treatment plans, potential side effects, and proper administration.
- Complete required Risk Evaluation and Mitigation Strategy (REMS) training and follow all associated program standard operating procedures to maintain regulatory and safety standards.
- Coordinate patient care with referring clinicians, addressing social, psychological, and physical needs that may require customized pharmacy services such as pre‑filled syringes or color‑coded labeling, and document these services so fulfillment and distribution teams can include all necessary materials.
- Serve as a key liaison between patients, caregivers, home health agency nurses, hospice nurses, and prescribing clinicians to ensure seamless communication and exceptional customer service for all stakeholders.
- Provide subcutaneous injection teaching and support for patients and caregivers, and coordinate appropriate nursing services with the prescribing clinician when needed.
- Provide on‑call coverage as directed by the Pharmacist‑in‑Charge to ensure patient needs are addressed outside of standard operational hours.
- Process prescriptions accurately and in a timely manner to support efficient downstream workflows for fulfillment, distribution, and patient support teams.
- Verify and document the discontinuation of services for patients by assigning appropriate therapy outcomes and deactivating patient charts, prescriptions, and software profiles so that patient support teams discontinue future refills appropriately.
- Implement and follow clinical management models for specific products and disease states to standardize and optimize patient care.
- Act as the primary intake contact for all new team referrals initiated by telephone, ensuring accurate information capture and smooth onboarding of new patients.
- Deliver consistently high levels of customer service in alignment with internal policies and expectations for exceptional patient and clinician experience.
- Build and maintain strong relationships with patients and clinicians, fostering trust and collaboration to support long‑term treatment success.
- Adhere strictly to company policies related to all government‑funded payers, including Medicaid and Medicare, ensuring compliance with applicable regulations and billing requirements.
- Dispense medications pursuant to valid physician orders and in full compliance with all applicable state, federal, and board of pharmacy laws.
- Ensure compliance with all internal company policies and procedures related to pharmacy operations and quality standards.
- Follow all federal and state laws governing the purchase, storage, and dispensing of controlled substances, including documentation and security requirements.
- Verify the accuracy of prescription order entry early in the workflow to identify and correct errors or discrepancies, improving efficiency and safety for subsequent processing steps.
- Perform final verification and quality assurance checks on dispensed prescription products to ensure accuracy, integrity, and patient safety.
- Assist pharmacy technicians with adjudication of pharmacy claims to ensure accurate communication and collection of payments.
- Monitor and support proper hazardous waste material management within the pharmacy, following safety and environmental regulations.
- Understand and supervise the aseptic technique used by pharmacy personnel, ensuring compliance with sterile compounding standards and quality control practices.
- Oversee pharmacy technicians, providing guidance, oversight, and support to maintain high standards of operational performance and patient care.
- Use patient education materials effectively during counseling and follow‑up to reinforce understanding and adherence.
- Perform intake of adverse events and product quality complaints and ensure appropriate documentation and reporting in accordance with regulatory and contractual obligations.
- Meet or exceed continuing education requirements set by the Board of Pharmacy and maintain current knowledge of clinical and operational best practices.
- Support the professional development goals of staff within the assigned patient care team by sharing knowledge, mentoring, and encouraging ongoing learning.
- Maintain an attitude of continuous learning and self‑education in pharmacy practice, clinical developments, and regulatory changes.
- Attend professional conferences and meetings with peers and share key learnings and topics with staff to support team development.
- Provide strong clinical oversight for pharmacy operations, ensuring that patient care, safety, and regulatory compliance remain top priorities.
- Engage in pharmacy management activities such as workflow optimization, compounding oversight, and quality control as needed to support the overall operation.
- Deliver high‑volume patient counseling by phone, typically 7 to 8 hours per day, while maintaining empathy, clarity, and professionalism in every interaction.
- Current and unrestricted pharmacist license in good standing in the state of Kentucky, with the ability to practice pharmacy.
- Residence within one hour driving distance of the Kentucky facility to support onsite work requirements.
- At least 2 to 3 years of direct patient counseling experience, including extensive phone‑based counseling.
- 3 to 5 years or more of post‑graduate experience in pharmacy operations, clinical oversight, and patient care.
- Demonstrated enjoyment of and comfort with counseling patients and being on the phone for 7 to 8 hours per day.
- Experience conducting comprehensive clinical reviews of pharmacotherapy, including drug utilization, adherence counseling, drug interaction screening, side effect profiling, and administration support.
- Proficiency in accurately processing, verifying, and dispensing prescriptions in compliance with state, federal, and board of pharmacy regulations.
- Working knowledge of sterile technique and related quality control issues, including supervision of aseptic practices.
- Strong oral and written communication skills, with the ability to explain complex medical information in clear, patient‑friendly language.
- Working knowledge of computers and excellent word processing skills for documentation and communication.
- Ability to adhere strictly to policies and regulations related to government‑funded payers such as Medicaid and Medicare.
- Ability to manage controlled substances in compliance with all federal and state laws.
- Capability to provide on‑call coverage as needed to support patients outside of regular business hours.
- Commitment to meeting or exceeding Board of Pharmacy continuing education requirements.
- Strong customer service orientation and commitment to delivering a high level of support to patients, caregivers, and clinicians.
- Graduate of an accredited school of pharmacy.
- Board Certified Oncology Pharmacist (BCOP) certification or willingness to obtain BCOP certification when eligible.
- Current pharmacist licensure in North Carolina and/or Texas, or the ability and willingness to obtain additional state licenses as needed.
- Experience coordinating care with referring clinicians and multidisciplinary teams, including social, psychological, and physical support services.
- Experience with customized pharmacy services such as pre‑filled syringes and color‑coded labeling for medication identification.
- Experience supervising pharmacy technicians and supporting their professional development.
- Experience with compounding and hazardous drug handling in a clinical pharmacy setting.
- Experience documenting adverse events and product quality complaints in compliance with regulatory and contractual requirements.
- Strong relationship‑building skills with patients, clinicians, and internal teams.
- Ability to effectively use patient education materials to support understanding and adherence.

This is an onsite position based at a Kentucky pharmacy facility, and candidates must live within one hour driving distance to support daily commuting. Training is typically conducted Monday through Friday from 9:00 a.m. to 6:00 p.m. Eastern Time, and ongoing work follows a similar daytime schedule, with additional on‑call responsibilities as needed to support patients outside of normal operating hours. The environment is a clinical pharmacy setting focused on high‑touch patient care, where pharmacists spend a significant portion of the day—often 7 to 8 hours—counseling patients by phone. The role involves working closely with a multidisciplinary team, including nurses, technicians, and prescribing clinicians, and using computer systems and word processing tools for documentation, order entry, and claim adjudication. Pay and BenefitsThe pay range for this position is $60.00 - $60.00/hr.
