What You'll Do: As a Registered Nurse at DSSW, you'll play a vital role in supporting individuals in home and community-based waiver programs. Your responsibilities will include: • Conducting initial assessments for clients transitioning off the CLASS waiting list • Performing post-hospital assessments to ensure continuity of care • Managing and completing annual renewal assessments • Collaborating with a compassionate team to ensure the highest quality of care What You Bring: • Current RN license in the State of Texas • Minimum 1 year of home care nursing experience • Strong typing skills (35 WPM with 80% accuracy) and Excel proficiency • A reliable vehicle , valid auto insurance, and willingness to travel throughout San Antonio • A heart for service and a commitment to empowering others Why Join DSSW?
