Job Details Job Location: Glendale, AZ (Southwest Phoenix area, including Glendale, Avondale, Goodyear, Laveen, and Peoria). Pay: $90,000 – $120,000 per year with potential signing bonus. Full‑time or part‑time. Work location: On the road. Responsibilities Assess patient’s home environment for safety hazards and architectural barriers, identify equipment needs. Identify functional problems and establish goals and a plan of care designed to improve the patient’s functional status, revising as necessary. Perform patient tests, measurements, and evaluations (e.g., range of motion, manual muscle tests, gait and functional analysis, body part measurement), record and evaluate findings to aid in establishing or revising specific treatment programs. Perform and complete start‑of‑care and ongoing assessments and associated paperwork in accordance with agency policy. Periodically re‑evaluate and upgrade the patient’s program to maximize functional improvement. Administer the exercise program utilizing specialized therapeutic exercise techniques and equipment. Plan and administer prescribed physical therapy treatment programs to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts. Recommend prosthetic/orthotic devices for patients or evaluate their devices and make minor adjustments or recommend modifications by the prosthetist/orthotist. Consult with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care. Perform and submit required documentation of patient evaluations, plan of care, clinical notes, progress reports, and summaries along with changes in orders. All patient visit notes must be turned in on the next scheduled workday, not to exceed forty‑eight (48) hours. Maintain positive internal and external customer service relationships. Display effective leadership skills, communication skills, reliability, and teamwork; plan and organize work effectively; represent the organization professionally; and demonstrate initiative and continual improvement. Education & Experience Graduate of an accredited Physical Therapy program and licensed by the state of Arizona. Previous experience in a clinical setting is preferred; motivated new graduates are welcome. Excellent communication and interpersonal skills. Ability to work both independently and collaboratively in a team setting. Strong commitment to patient‑centered care.
