Role Overview Provides hands-on nursing support to promote the health and wellness of residents in licensed facilities, including attending Community Participation Supports programs. Key Responsibilities Deliver compassionate care aligned with the mission to care, comfort, and heal for adults with intellectual disabilities. Integrate healthcare with support services to enhance residents' quality of life. Facilitate communication with internal and external stakeholders and ensure proper documentation using Electronic Medical Record systems. Qualifications & Skills Licensed Practical Nurse (LPN) with 3+ years' experience, especially with tracheostomy and Type 1 Diabetes Experience working with individuals with intellectual disabilities Basic computer literacy and EMR proficiency Excellent communication and interpersonal skills Valid PA driver’s license and satisfactory background checks, including criminal and FBI clearances Additional Information Committed to equal opportunity employment, promoting a diverse and inclusive work environment. #J-18808-Ljbffr
