Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Nellas. This full-time opportunity offers the chance to provide individualized care while helping residents improve mobility, rebuild strength, and work toward greater functional independence.
What You'll Do

• Complete physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment focused on strength, gait, balance, transfers, endurance, and functional mobility
• Establish measurable rehabilitation goals and track resident progress
• Adjust treatment approaches based on clinical findings and response to therapy
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies
• Maintain accurate and timely clinical documentation
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team

Qualifications

• Active West Virginia Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical judgment, communication, and teamwork skills
• Skilled nursing or rehabilitation experience preferred, but not required
• New graduates are welcome to apply
