Description
  • Conduct thorough evaluations of patients in both inpatient and outpatient environments to create personalized treatment plans
  • Implement physical therapy techniques aimed at enhancing mobility, strength, balance, and daily function
  • Work closely with a multidimensional team of nurses, physicians, case managers, and rehabilitation specialists to achieve optimal patient outcomes
  • Guide and inform patients and their families about therapeutic goals, discharge procedures, and necessary home exercises
  • Maintain accurate documentation of evaluations, treatments, and patient progress adhering to facility standards and regulations
  • Engage in collaborative care team meetings and conferences to ensure cohesive patient care
Education
  • Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
  • State licensure to practice as a Physical Therapist
  • CPR certification preferred
  • Additional specialized certifications (e.g., geriatric, neurological) are advantageous but not mandatory
Skills
  • Preferred previous experience in a similar role
  • Able to effectively treat patients with complex medical and mobility challenges
  • Excellent communication and collaborative abilities
  • Proficient with digital charting and clinical documentation systems
