Field-Based Community Health Advocate

UnitedHealth Group (OptumCare) in Las Vegas is seeking a Field-Based Community Health Worker to identify social needs, coordinate referrals, and support Medicare/Medicaid members. You will visit members in their homes, document progress, and collaborate with the care team to improve health outcomes. The role requires a high school diploma, basic computer skills, and local travel.
