PRN Physical Therapist - Skilled Nursing

We are seeking a Physical Therapist to join our team in a PRN or Part-Time capacity. In this role, you'll create personalized, patient-centered treatment plans that address each resident's unique physical, functional, and mobility needs. What You'll Do Restore Independence: Develop and implement individualized treatment goals to improve strength, mobility, balance, and daily living activities. Educate & Support: Train patients, families, and caregivers on treatment plans, safe mobility techniques, and progress. Evaluate & Document: Conduct thorough assessments and maintain accurate, timely documentation of each resident's progress. Collaborate Effectively: Serve as a key communicator between the clinical team and the patient's support network. What You'll Need Active Physical Therapist (PT) license in the state. Previous experience in a Skilled Nursing Facility is preferred. Strong verbal and written communication skills to advocate for your patients.
