Registered Nurse Case Manager
UnityPoint Health - Proctor
UnityPoint Health Marshalltown Hospital is seeking an experienced RN Case Manager to coordinate patient care across the continuum, focusing on discharge planning, utilization review, and interdisciplinary collaboration to support safe transitions and cost-effective care. This part-time role (28 hours per week) runs 8:00 AM–4:30 PM with occasional weekend coverage and 1–2 holidays per year. You will work closely with providers, nursing staff, social services, patients and families to guide care #J-18808-Ljbffr
