Hospice Registered Nurse (RN)

Location: Moab, Utah

Assignment Duration: This is a 13-week contract position.

Job Description: We are committed to providing compassionate, high-quality end-of-life care to our patients and their families. We believe in treating every patient with dignity and respect in their preferred place of care, usually in their home. The Hospice Registered Nurse (RN) will provide skilled nursing care and support to terminally ill patients and their families in accordance with the patient's plan of care and hospice philosophy.

Responsibilities:

• Perform comprehensive assessments of patients' needs and coordinate individualized care plans.
• Provide pain and symptom management consistent with the hospice plan of care.
• Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy.
• Collaborate with the interdisciplinary team including physicians, social workers, chaplains, and volunteers.
• Participate in on-call rotation.
• Maintain accurate and timely documentation in compliance with state, federal, and organizational policies.

Required Experience / Certifications / Licensure:

• Current Registered Nurse (RN) license in the state of Utah or compact state RN license.
• Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
• Strong clinical assessment skills.
• Excellent communication, interpersonal, and organizational skills.
• Compassionate demeanor with the ability to support patients and families during difficult times.
• Valid driver's license and reliable transportation required for home visits.
• Current BLS Certification.
• Familiarity with Hospicesoft/careficient EMR preferred.
