Hospice Aide I - Crisis Care

This position applies the nursing process in providing direct and indirect holistic care to patients and their families. This position collaborates, in a collegial manner, with physicians and other health care team members in meeting the patient's needs, and may delegate certain aspects of care to those under their direct supervision, within the scope of nursing practice. This position requires weekend hours and may be on-call during the evenings and nights. Thus, the position must be flexible.

Job responsibilities include performing initial and ongoing assessment of patient and family, communicating assessment findings to other health care providers, planning care for patient and family based on assessment, standards of care, and optimal specific outcomes, and updating current plan of care as needed based on patient/family input and healthcare needs.

Working conditions include climbing, concentrating, continuous learning, hearing, interpersonal communication, kneeling, lifting, pulling, pushing, reaching, reading, sitting, standing, talking, thinking/reasoning, use of hands, color vision, visual acuity, and walking.
