Hospice RN Case Manager

Registered Nurse Case Manager We are seeking a Registered Nurse Case Manager to oversee hospice patient care, develop and implement personalized Plans of Care, and evaluate patient needs regularly. Responsibilities conducting initial assessments within 48 hours of referral identifying physical, psychosocial, and environmental needs coordinating care plans documenting patient responses collaborating with patients, families, physicians, and the interdisciplinary team Qualifications current RN license, at least one year of clinical experience, CPR certification, a valid driver’s license, and reliable transportation. Preferred Skills independence, strong communication, organizational abilities, and effective written skills.
