Home Health And Home Care Team Member You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Expect to be hands-on providing personal care, supporting daily living needs, and making a real difference in someone's day, every shift. This is a role for someone dependable, patient, and service-driven who wants meaningful work and flexible opportunities. Key Responsibilities Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan Prepare meals and assist with feeding as needed Support medication self-administration through reminders (no independent med administration) Monitor and report patient condition and changes to the supervising nurse Maintain a clean, safe, and orderly client environment Assist with linens and light client laundry as appropriate Use safety equipment properly (rails, supports, stools, etc.) Document care provided according to company and regulatory standards Travel reliably to patient homes and assigned visits across your service area Complete required in-service training Must-Have Requirements Age 18 or older Active HHA or CNA certification or willingness to obtain certification or 1 year experience is the last 2 years Willingness to complete competency and skills assessments Ability to follow written care instructions and document services Compassion, patience, and professionalism with vulnerable populations Ability to work independently in home settings Reliable transportation and ability to travel to client homes Ability to lift up to 25 pounds Willingness to complete background screening, and onboarding requirements If you're ready to make a daily difference in someone's life we'd love to meet you.
