What You'll Do The Clinical Supervisor - Therapy is responsible for ensuring that patient care is coordinated and managed appropriately. They serve as the first point of contact for field clinicians to ensure support is easily accessible in a timely fashion. The supervisor is responsible for ensuring that care and services are delivered appropriately and for supervising clinical personnel. This is a leadership position that oversees clinicians providing patient care in patients’ residences. This is a hybrid role with field work and on-site work at the 21HHS office. Two days in the Roseville or Natomas office (TBD). Three days in the field: 12 patient visits each week (supervisory visits of field clinicians). Key responsibilities include: In-field oversight of clinicians who provide care and treatment to patients at their homes. Handle incoming calls timely and follow up to meet internal and external customer needs. Coordinate training and educational programs for nurses within the field to improve outcomes and competencies. Assess staff learning needs and formulate specific plans and goals for the next evaluation period; perform administrative call duties and serve as a role model and resource for staff. Act as the first point of contact for field clinicians to assist with patient care escalations. Coordinate at least bi-weekly case conferences with the clinical team to discuss patient progress and revise the care plan as needed. Review and evaluate each case by reviewing services provided by clinicians, conferences, record review, and discuss and verify impressions; instruct and guide clinicians to promote more effective performance and delivery of quality home care services. Review patient’s clinical diagnosis, medications, procedures, and clinical course. Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing a plan of care. To Be a Success At least two years of management or supervisory experience in a rehabilitation therapy department or program, preferably in home care. Degree from a baccalaureate or master’s program in an applicable discipline, approved by an accredited organization. Current California Physical Therapist license. Current Basic Life Support (BLS) certification. Valid California driver’s license with an acceptable driving record. All licenses and certifications must be current at time of hire and sustained throughout employment. Benefits Wage range: $143,000.00 - $163,000.00 plus a generous bonus paid quarterly.
