We are currently seeking a dedicated and passionate Care Manager to join our team during our 3rd shift and weekends.

**Job Summary:**
The Care Manager will provide exceptional support and care to our clients during the overnight hours and weekends. This role involves coordinating and delivering clinical and non-clinical services to meet the individual needs of clients, ensuring their safety, comfort, and overall satisfaction. The Care Manager will also collaborate with families, healthcare professionals, and community resources to promote positive outcomes for those in our care.

**Key Responsibilities:**
- **Client Care:** Assess and identify clients’ needs through direct observation and interaction, providing personalized care and support.
- **Coordination:** Collaborate with healthcare providers, family members, and interdisciplinary teams to create and implement individualized care plans.
- **Monitoring:** Regularly monitor clients’ health status, documenting changes and communicating effectively with relevant parties.
- **Support:** Offer emotional and psychological support to clients and their families, addressing any concerns or questions they may have.
- **Crisis Management:** Respond to emergency situations with professionalism and care, employing conflict resolution and de-escalation techniques as necessary.
- **Documentation:** Maintain accurate and up-to-date client records, ensuring compliance with regulatory standards and internal policies.
- **Education:** Educate clients and families on care techniques, health management, and available resources to enhance their understanding and involvement in care.
- **Team Collaboration:** Work closely with other staff members to create a positive and supportive environment that prioritizes client-centered care.

**Qualifications:**
- **Education:** Bachelor’s degree in Nursing, Social Work, Psychology, or a related field preferred. Relevant certifications and licenses (e.g., RN, LPN, Care Manager Certification) are a plus.
- **Experience:** Previous experience in care management, nursing, or a related field, particularly in overnight or weekend settings is highly desirable.
- **Skills:** Strong interpersonal and communication skills, with the ability to build rapport with clients and families.
- **Technology Proficiency:** Experience working with electronic health records (EHR) and other healthcare technology.
- **Certifications:** CPR and First Aid certified.
- **Availability:** Ability to work overnight shifts and weekends consistently.
