Duties include but are not limited to:

• Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction while functioning as a key team member providing comprehensive, coordinated nursing care to panels of patients.
• Demonstrates leadership in delivering proactive, patient-centered care by using nursing knowledge, evidence-based guidelines, and standardized nursing protocols within the immediate practice setting.
• Acts as the primary coordinator for a designated panel of patients, emphasizing health promotion, chronic care management, patient self-management support, and overall wellness.
• Collaborate with the PACT team members, extended care support team, and external services to develop and implement holistic, patient-driven care, facilitate care transitions, and ensure timely, coordinated access to health care resources.
• Engages in patient and family education and advocates for patients' needs to enhance patient outcomes and wellness.
• Champions a collaborative team approach for managing patient panels, ensuring continuity and quality of care tailored to individual patient goals and needs.
• Leading professional practice, integration of evidence-based practice, quality improvement, and resource allocation to improve outcomes for the immediate practice setting.
• Other duties as assigned.

Dimensions of Nursing Practice:

Leads professional practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) to improve evidence-based and/or data-driven outcomes for an identified patient population and/or specialized areas within immediate practice setting.

Veteran/Patient Driven Care:

Improves Veteran/patient driven care by optimizing the patient experience, satisfaction and/or safety for the immediate practice setting.

Leadership:

Communicates and collaborates as an effective leader of the interprofessional team, at the expert level, with improved evidence-based and/or data-driven outcomes for the immediate practice setting.

Professional Development:

Expands knowledge base and practice evaluation through mentoring, precepting, and/or teaching to improve outcomes in the immediate practice setting.

Evidence-Based Practice/Research:

Leads the integration of evidence-based practice/research with clinical expertise to improve quality of care and resource utilization with improved outcomes for the immediate practice setting.

Licensure:

1 full and unrestricted license from any US State or territory

Work Schedule:

Intermittent - Tour of Duty: could be during work week Monday - Friday, occasional Saturday Coverage from 7:30am - 4:00pm. Incumbent should provide at least 2-3 days per week and/or anticipate increased days around critical shortage dates (example: federal holidays/season breaks (spring/summer/fall winter).
