This is a hybrid position (2 days remote 3 days in the field) giving you the flexibility to work from home while building strong in-person connections with members across San Diego County.

Key Responsibilities

• Serve as the primary point of contact for members and coordinate their care
  
• Develop and manage personalized care plans with members and providers
  
• Collaborate with healthcare teams and community resources
  
• Support members through education advocacy and engagement
  
• Identify and address gaps in care
  
• Maintain accurate timely documentation
  

Requirements

• Experience in care or case management (community-based preferred)
  
• Knowledge of Medi-Cal and ECM strongly preferred
  
• Strong communication and interpersonal skills
  
• Ability to work in a team-oriented environment
  
• Willingness to travel within Orange County
  
• Bilingual (English/Spanish) preferred
