SNP Care Navigator — Telephonic Health Manager
CVS Health Corporation
CVS Health Corporation is seeking a Telephonic Care Manager Specialist to coordinate SNP member care, including those with social determinants of health, through annual health assessments and closing care gaps. You will collaborate with the Registered Nurse Care Manager, Care Coordinator, Social Worker and interdisciplinary team to develop personalized care plans, document activities in the EHR, and ensure regulatory compliance while delivering compassionate, high-quality care. #J-18808-Ljbffr CVS Health Corporation
