Value-Based Primary Care NP — Independent Clinician

Oak Street Health in Warwick, RI is seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective primary care to adults on Medicare within a value-based model. You will work on a collaborative team with physicians, NPs/PAs, and care managers to ensure seamless, high-quality care. Ideal candidates hold an MSN, DNP or NP certificate, are licensed in RI, and have DEA credentials.
